Home Health enables a whole team of health care professionals, as needed to treat homebound patients without the daily expense of a hospital room. Patients may now have a choice about the level of care they receive and where it is provided.

Minimum patient requirements for Home Health Services are:

• Patient must be homebound and require skilled services.
• Services must be medically necessary and intermittent.
• Care plan must be overseen by a physician.
